On-Line View of Paystubs

An Employee Self-Service web site allows you the comfort of viewing your pay stubs, payroll information, and W-2 tax forms wherever you are. Access Cards are for those who, for whatever reason, do not have a checking/savings account, but would like to enjoy the benefits of having an ATM/Debit card, with a Visa logo. Paychex would act as the bank and the ATM/Debit Card would access your funds. Visit https://eservices.paychex.com/secure.

Pay Period

As the work week ends on Sunday, hours are to be called in/faxed/emailed by 10:00am Monday morning. Checks are cut weekly and mailed out on Friday. A time card with a manager's signature must be received by Elite Technical before the check can be sent out.

Premium Only Plan (POP)

Elite Technical sponsors a Section 125 Premium Only Plan. This IRS-approved plan allows Elite Technical employees to make deductions for their 401(k) contributions, their health/dental benefits, and some supplemental insurance premiums, before taxes. Reducing taxable earnings leads to less taxes being taken out on a weekly basis.

401(k) Plan

Elite Technical has established a 401(k) program with Oppenheimer Investments, and Paychex, our payroll company, does the record-keeping. Our plan allows the participant to save up to 96% of his/her salary on a tax-deferred basis, up to the dollar limit set each year by the IRS. A wide selection of investment choices is available. Entry dates are the first day of each new month and enrollment can be done online or by phone. There are no eligibility requirements, other than participants must be over the age of 21 and have received at least one paycheck from Paychex to be sure they are in the system. Account balance inquiries, loan information, and change investment selection options are available to the participant daily by calling Paychex Automated Phone System at 1-800-401-3999 or visiting their web site at: https://benefits.paychex.com. Health Insurance

Elite Technical offers its eligible employees a voluntary group health insurance plan, through Empire BlueCross BlueShield. The effective date of coverage is the first of the month following 30 days of work. It is the Empire Prism EPO plan, and has In-Network only. A Summary Plan Description outlining coverage may be obtained from the Human Resources Department. Visit http://www.empireblue.com to see if your providers are listed. No referrals necessary in order to see a specialist.

Dental Insurance

Elite Technical offers a first class, voluntary group dental insurance plan, through The Guardian Life Insurance Company of America. Guardian has two plans: a PPO and a Pre-Paid HMO. The PPO plan lets you visit any dentist, but usually provides better benefits for the services of the PPO network dentists. It covers 100% Preventative Care, 80% Basic Care, and 50% Major Care. Orthodontia is not covered with the PPO plan, but is covered with the Pre-Paid Plan. The Pre-Paid Plan costs less, but requires that you visit a network dentist only. The directory is smaller, but benefits are unlimited. You pay a fixed copay to the dentist for each service performed.

Short Term Disability

An employee who suffers an off-the-job injury or illness may be entitled to benefits from the state. Benefits may vary from state to state. Please consult with the Human Resources Department for further information. Employees may choose to purchase additional coverage.

Workers' Compensation

An employee may be eligible for state workers' compensation benefits for job-related injuries. If you are injured while performing a job-related duty, you should immediately report the accident to your supervisor. Call the Hartford Claim Center for your area. Give our policy # 83WECTC2277. Explain your injury and receive a claim number. Go to any doctor/lab/hospital and tell them this is a Workers Comp. injury and give your claim number. They will file the necessary paperwork. If your injury results in absence from work, a letter from a doctor stating that you are physically well enough to return to work is required prior to, or on, the date of return.

AFLAC

Additionally, supplemental insurance plans are offered by the company through AFLAC. Contact the HR Dept. for more information.
